I have a sample app on apex.oracle.com where I do not see page types like 'Report and Form', 'Master Detail' etc.

I have noticed that these options are available if I create a new sample app and then create first page during that app creation wizard. But, once I have created the app (committed) and then I go thru Create Page, it does not show me those page types.

Any idea what could be causing this?

You are, I guess.

These page types are not visible at the first page type choice level. They are available at the second level:

               (Level 1)        (Level 2)   
Create Page -> Form          ->  Form on a table with report
                          or ->  Master detail form
                          or     ...
